What is the RELEX SMILE technique ?

How much time is needed for the concern of the patient to turn into a smile?


The RELEX, is a completely new technique for correction of vision, through the use of femtosecond lasers. It is the result of a revolutionary principle, by the ZEISS researchers. It involves removal of a small biconvex lens sculpted intact by the femtosecond laser in corneal depth. The volume and shape of this lens depends on the correction of the visual defect. The lens is thicker at the center and its extraction flattens the cornea by decreasing the radius of curvature. It can be done using the femtosecond laser VISUMAX of Carl Zeiss Meditec.

This is an exclusives procedure asthe lasers of other brands are not aware of this technology. The VISUMAX is a new type of femtolaser, which projects a generation ahead of its competitors, making them outdated. We have been using this technology since 2011.

It allows us to perform a correction of myopia throughout the femtolaser without having to split the operation into two parts as is commonly done with FEMTOLASIK:

- The first groove of the corneal flap with the femtolaser,

- The second of myopia correction with the excimer laser.

The RELEX is divided into two types of intervention, the FLEX and SMILE.


SMILE & FLEX techniques

In the FLEX the correction of myopia and astigmatism takes place, flattening the corneal curvature.


The SMILE is still a step ahead of the FLEX.

Who can be operated with SMILE technique?

The inclusion criteria are:

Myopia up to 10 diopters

The SMILE is definitely the best technique for athletes, because the cornea remains virtually unscathed and you do not need a break .


What is refractive surgery?

The cornea is the front, transparent eye.

It is the first of the two lenses of the eye, as if it were the first of the objectives of a camera.

The refractive surgery reshapes the surface, modifying the radius of curvature, in order to increase or decrease the refractive power of the cornea.

In this way the defects of pre-existing views can be cancelled. This type of treatment involves the use of an excimer laser, which emits a beam of

coherent light in the ultraviolet spectrum to vaporize some tens of micron (thousandths of a millimeter) of corneal tissue. Everything happens with a precision and absolute repeatability, not obtainable with other methods.

There are other surgical techniques that relate to particular cases in which the laser is not indicated. You can then insert folding soft lenses within the eye, in order to also correct the defects of higher sight with great accuracy and as much satisfaction.

Only a careful examination done with the help ofthe required specialized tests, prescribed by an eye doctor can determine the personalized choice of the type of intervention.

The methods described below use excimer laser to change the curvature of the cornea.

They differ in their approach to the deep tissue (corneal stroma) where it uses the excimer.


PRK

PRK (Photo Refractive Keratectomy) involves the removal of the superficial corneal tissue (epithelium) with a blunt spatula to allow the use of the excimer laser on the underlying tissue (stroma).

The epithelium re-grows and closes the corneal abrasion in 3-4 days andfor the purpose of mitigating the pain, a protective contact lens is applied.

During this period we recommend rest at home and taking pain medication for a few days

LASIK

LASIK (Las assisted er I n situ eratomileusis K) differs from PRK, because the approach to the deep plane of the cornea, where the excimer laser will have to exercise its action is guaranteed by a razor blade cutting, a round blade or corneal flap, which opens as the porthole of a washing machine or a ship.

The microkeratome creates a corneal flap. This is done with the use of a a scalpel blade mounted on a mechanical device with gears, which, with a go and come mechanism, mechanically cuts a superficial corneal layer to create thecorneal flap. Only after it has been raised, the excimer laser can act on the underlying stroma.

It is a LASIK WITHOUT SCALPEL.

It differs from LASIK because the washer or flapcorneale is separated gently from the floor below by the expansion of a series of gas microbubbles, created in the depth of the corneal tissue with a custom design for each eye, conceived and performed by a complex management program.

Femtosecond laser

It is the newest refractive surgery, the safest and most effective technique.

The femtosecond laser is a new laser for eye surgery The name is related to the pulse duration of femtoseconds (1 femtosecond = 10 to - 15 seconds). 

For a comparison, you may remember that one femtosecond is the time required for an electron to move from one atom to another.

Recalling the formula, Power = Energy / Time, it's easy to understand that, thanks to a pulse duration so short, a few femtosecond can be used on the cornea for very low levels of energy.

Thanks to this innovative method we are able to obtain a more precise cut and increase the level of safety of the surgery. The evolution over time has led today to Femtosecond Laser Generation IV.

This intrastromal laser got FDA (Food & Drug Administration, the federal oversight body of the United States) approval in December 1999.


WITHOUT SCALPEL, WITHOUT PAIN

The treatment is carried out under the guidance of an operating microscope combined with femtosecond laser, controlled by a computer.

This makes it possible to act exclusively on the corneal plane, leaving the corneal tissue intact.

Once the optimal level of depth is reached, the laser beam generates a series of micro gas bubbles, close to each other, which, by expanding, gently separate the tissues, without producing heat and are able to perform any geometry of cutting .

Eye surgery

Lying on your ophthalmologist chair you will be placed under a coaxial surgical microscope to the femtosecond laser, controlled by an accurate and personalized e-management program.

The treatment is performed within minutes.

The duration of the pulses of the femtolaser is a maximum of 22 seconds per eye.

After turning the table 180 degrees you undergo the action of the excimer laser, coaxial to a second operating microscope, which is managed by a specially developed and patented software.

The corneal flap created by femtosecond laser is raised to proceed to treatment with excimer laser with an average duration of less than one minute.


Why is it better to create a blade with a femtosecond laser cut with a microkeratome?

A microkeratome is a scalpel mounted on a mechanical device with gears that move forward and backward, cutting the cornea & creating a superficial corneal tissue lamella.

It is capable of working only on a horizontal plane, such as a planer, while the cornea is curved. Also you can meet all the drawbacks of a complex system of gears, as in high watchmaking.

The femtolaser used at Dr. PANIC is the only one that can perform cutting geometry significantly higher than the blades of the microblades/ The ZEISS has recently developed a new procedure for the correction of refractive defects with the exclusive use of Femtosecond Laser called RELEX . Itis divided into: FLEX and SMILE.

The technique used in the center of Dr. PANIC is the most recommended technology worldwide and is used by NASA for its astronauts and authorized by the US military and security bodies and provides guaranteed results.

It does not involve an actual cutting of the cornea, only generates a single layer of gas microbubbles.

The treatment can be stopped at any time and be resumed after a few minutes or after several days.

AFTER SURGERY

With the femtolaser technique you may experience discomfort and watery eyes for a few hours after surgery. Generally, the day after, daily activities can be resumed. If the treatment is carried out on Friday, youcan return to work three days later, i.e. on Monday.

With PRK, however, you must use protective contact lenses for five days and take pain medication as you can have pain for 24-48 hours, accompanied by eye irritation and watery eyes. It is advisable to return to work no earlier than 10-14 days.
